Active Member
Choose this category if you are: renewing your ISCFS membership and want to retain "Active" category status; upgrading from Associate or Corresponding Member status, including Research and Craniofacial Orthodontic Corresponding members; applying for New membership in the "Active" category. New members must ALSO select "New Member" below.
Nb. Membership category upgrade to "Active" status requires payment of $50 Application Fee and submission of Application Form and full Dossier, in addition to upgrade-category dues payment. If upgrading, you must ALSO select "Renewing member upgrading category" below.

Associate Member
Select this membership category if you are currently an Associate ISCFS member or want to upgrade to "Associate" member status from "Corresponding" membership (including current Research and Craniofacial members), or if you are a New member applying for Associate membership. (Associate membership privileges are limited.)

New Member

Renewing Member, upgrading category
Upgrading your ISCFS membership status to a new category requires payment of the $50 Application Fee AND submission, by mail, of the downloadable Application Form AND full Dossier for the NEW category. Upgrades will not be processed until Fee is paid and Dossier has been received by mail and approved. You must ALSO select your new membership Category above.
You must verify your Dossier submission by selecting "Yes" on the order form drop-down menu for this item.

Renewing Member, same category
Select this option if you are currently an ISCFS Active, Associate, or Corresponding member in good standing, and you want to renew your membership in your current category.
If you are upgrading your membership category, you must select "Renewing member, upgrading category" instead, AND you must select the new category above. Nb. Membership category upgrades require new category Application and Fee payment as instructed.
